For participating, you will have a chance to win a $250 Teva gift card. You are not required to enter the drawing. Once you reach the “almost done” page, close the browser not to enter the drawing.

How to Take the Teva Guest Survey

You Must:

• Have access to a computer and Internet access.
• Be able to read English, Spanish or French.
• Have your recent Teva receipt that contains an invitation for the survey.
• Be 18 years of age or older to participate.

Take By Online:

1. Go to www.Tevalistens.com.
2. Select whether you visited a store, placed an online order, or placed a telephone order.
3. Select the location of where you shopped.
4. Provide the company with your recommendation (would you recommend the store).
5. Depending on how you shopped, your questions may be different. However, they will all focus on rating your shopping experience. For example, if you shopped in store, you may be asked if you were welcomed upon entering. If you shopped online, you may be asked if the website was easy to navigate. You will be asked about the store, the sales team and the stuff you purchased. Most of the questions will be based on the scale of one to five or one to 10, though some may provide a multiple-choice option.
6. For the next to last question, you will be asked about your thoughts. Please use the text box to provide your answer. Write about anything you want that relates to the store.
7. Enter for the gift card drawing. Enter your name, email address and telephone number.
