The information from the questionnaire is used by the company to make any number of improvements. This survey is done online for your convenience.

Taking this survey gives you a voice. You have an opportunity to tell the company how you really feel. In many cases, customers want to give this information and may not know how; this gives them a way to tell the company. The company wants the information because it gives them an idea of what needs improvement. They will then work on making the restaurant better to give you a great experience. How to Take the Fazoli’s Guest Experience Survey


You Must:

  • Have access to a computer and Internet access.
  • Be able to read English or Spanish.
  • Have your recent Fazoli’s receipt that contains an invitation for the survey.

Take By Online:

  1. Go to www.TellFazolis.com.
  2. Enter the four-digit store number, time and date from your receipt. These are numbered in red on a diagram to help you. Click Start.
  3. Answer questions. These questions are meant to help the company learn about your experience.
  4. They will touch on many areas of the store, such as cleanliness and service. Make sure to follow directions and be as honest as you can. If you rush through the survey, it will not be as helpful.
  5. Make the offer valid. To validate the offer, write the validation code you receive on your receipt and take it with you. You must purchase a drink in order to receive the free entree.
