The Pizza Insight Guest Survey, found at www.Pizzafeedback.com, is an online questionnaire designed by Pizza Insight to help improve customer satisfaction. You have a chance to enter a drawing for a $50 gift certificate to Pizza Insight.

How to Take the Pizza Insight Guest Survey

You Will Need:

• Have access to a computer and Internet access.
• Be able to read English.
• Have your recent receipt that contains an invitation for the survey.
• Be 18 years of age or older to participate.

Take By Online:

1. Go to www.Pizzafeedback.com.
2. Read about the survey and the prize and click Next.
3. From the drop-down boxes, enter the state, city and restaurant. You can also enter the date and time of visit, though those are optional.
4. Select whether you dined in, carried out or had your food delivered.
5. Answer if you would recommend the restaurant.
6. Answer questions about the team using a five-point scale.
7. Answer questions about the restaurant using the same scale.
8. Answer questions about the food and drink.
9. Select one answer about how they did overall.
10. Type in any thoughts, ideas or comments you have. This could include any problems you had, ideas for new ingredients or items, and other general comments. You can also use this spot to commend the staff on a job well done.
11. If you would like to have a reply based on your comments and to enter the drawing, you should enter your name, daytime telephone number and email address. All information must be given. However, if you don’t care about receiving a reply to your comments and if you don’t want to enter the drawing, you can simply close the browser window.
12. You can also check the box (which is already checked) if you would like information about specific things. Click Done when finished.
